Lakeside Center for Rehabilitation and Healing is a 122-bed skilled nursing facility that offers short-term rehabilitation, long term care, palliative care, hospice services and outpatient rehabilitation. 

It is our mission to provide our residents high quality care while promoting their optimum health and well-being. LCRH can achieve our resident’s highest level of function with our interdisciplinary team that approaches the health and well-being of each resident at all levels. 

We are conveniently located in North Jacksonville of Dunn Avenue and Armsdale Road. 

Our experienced and caring interdisciplinary team of physicians, nurses, nutritionists, social workers, physical, occupational and speech therapists are committed to achieving and maintaining the maximum health potential for each of our patients.Our team will assist in a smooth transition for our short-term patients returning home.

We invite the client and family to participate when generating a plan of care that is appropriate for each patient. Our care team is experienced in a wide variety of complicated medical problems including diabetes, cardiac problems, wound care, post-surgical care, orthopedic rehabilitation, gastrointestinal disorders including those that require tube feeding and many others.

Our experienced staff can also offer your loved one care for many medical problems other facilities in our area don’t offer. These include new tracheostomy care, wound care including wound vacs and total parental nutrition.

Our facility invites family and friends to visit throughout the day and pre-screened pets are welcome. We offer a wide variety of onsite activities and socials so we have something to suit each individual’s needs and interests and we invite family and friends to participate in our social activities with their loved ones.
